For patients Par La Ville Dental may use the Personal Information it collects for purposes such as:

• To process patient’s enrollments applications
• To send out patient appointments reminders, send out claims to insurance companies,
• process payments and issue receipts
• To respond to your inquiries
• For purposes for which the applicant provides consent
• For purposes permitted or required by law.

When and to whom does Par La Ville Dental Ltd. disclose Personal Information of patients?

Par La Ville Dental Ltd. does not sell any Personal Information to anyone for any purpose. Par La Ville Dental Ltd. does not disclose the Personal Information it collects to third parties other than the following:

• Par La Ville Dental Ltd will disclose personal information to offices/ Doctors to whom you are being referred to for treatment not performed at Par La Ville Dental Ltd. Par La Ville Dental will provide other dental organizations with contact information, Insurance information and other distinguishing data to determine service that needs to be provided.
• Par La Vile Dental discloses Personal Information for such other purposes as a patient may consent, or as permitted or required by law.

You may, subject to legal restrictions, withdraw your consent to the further use or disclosure by Par La Ville Dental Ltd. for some or all of the purposes identified (although this may mean that you will not be able to benefit from certain services provided by Par La Ville Dental Ltd.  Contact the Par La Ville Dental Ltd. to note any objections.

How accurate is the Personal Information held by Par La Ville Dental Ltd.?

Video surveillance at ODA premises

Par La Ville Dental Ltd uses video surveillance at its premises for the purposes of ensuring a safe and secure workplace. Video cameras are located in the waiting room, above the reception desk, in our hallway, Office Managers office and staff room (on Mute) only.
